چکیده

l aser photocoagulation continues to play a prominent role in the treatment of diabetic macular edema (DME) despite the introduction of anti-VEGF medications. Although traditional continuous-wave laser mode has proved to be effective for DME, as shown in the Early Treatment for Diabetic Retinopathy Study1 there are a number of drawbacks including, but not limited to, permanent destruction of the photoreceptors and enlargement of laser scars into the fovea with further deterioration of vision. We are now learning that modified treatment parameters and selective laser wavelengths have the ability to produce a less destructive and more therapeutic effect.

Evaluation of Changes of Macular Thickness in Diabetic Retinopathy after Cataract Surgery

PURPOSE To assess the macular thickness changes after cataract surgery in diabetic patients using optical coherence tomography (OCT). METHODS We retrospectively reviewed the records of 104 diabetic patients who underwent cataract surgery. We examined the changes of macular thickness using OCT before cataract surgery and 1 week, 1-, 2- and 6-months after surgery. The central subfield mean thic...

Electronic Patient Records to Identify Patients in the United Kingdom with Diabetic Macular Oedema Suitable for ILUVIEN® (Fluocinolone Acetonide)

INTRODUCTION We describe a proactive method using electronic patient records (EPR) to identify pseudophakic patients with diabetic macular oedema (DMO) that might benefit from treatment with 0.2 µg/day fluocinolone acetonide (FAc; ILUVIEN(®)) implant. METHODS Our EPR audit tool (Medisoft(®)) identified diabetic patients (May 2011-December 2014) with National Screening Committee-confirmed grad...

Laser-Based Strategies to Treat Diabetic Macular Edema: History and New Promising Therapies

Diabetic macular edema (DME) is the main cause of visual impairment in diabetic patients. The management of DME is complex and often various treatment approaches are needed. At the present time, despite the enthusiasm for evaluating several new treatments for DME, including the intravitreal pharmacologic therapies (e.g., corticosteroids and anti-VEGF drugs), laser photocoagulation still remains...

Combination of Anti-VEGF and Laser Photocoagulation for Diabetic Macular Edema: A Review

Diabetic macular edema (DME) is the most common cause of vision loss in diabetic patients. Thirty years ago, the Early Treatment Diabetic Retinopathy Study (ETDRS) demonstrated that focal/grid laser photocoagulation reduces moderate vision loss from DME by 50% or more; thus, macular photocoagulation became the gold standard treatment for DME. However, with the development of anti-VEGF drugs (be...

OCT Classification of Clinically Significant Macular Edema

Diabetic macular edema is the commonest cause of visual loss in patients with non proliferative diabetic retinopathy and a common cause of visual loss in PDR. According to Early Treatment Diabetic Retinopathy study, early detection and laser treatment of clinically significant macular oedema decreases the risk of moderate visual loss by 50%. Though laser has been the standard of care till recen...
